COVID-19 Patient Registry

This registry has been created to capture COVID-19 patients’ information to provide researchers and decision-makers more accurate information on patients’ status and well-being.

​One of the biggest challenges we’ve observed is that there’s no single database that captures information on COVID-19 patients from around the world.

Our mission is to break the silos and have better access to COVID-19 patients data, so we researchers can better understand the characteristics of this virus and decision-makers can make data-driven decisions, instead of relying on blind models.

​This survey is for all COVID-19 confirmed patients or COVID-19 patients who developed symptoms but have not been tested positive for the virus.

The survey should take about 2-3 minutes to complete. Patient identifiers such as name or date of birth will not be collected. All de-identified information will be kept strictly confidential and will only be shared with researchers compiling information if the patient agrees to be contacted in the future.

The success of this collaborative effort depends on active participation by patients from around the world. We ask that you share this site with your friends and family and encourage them to report any cases of COVID-19.
